Withdrawal Requests: If your plans change and you need to withdraw from the Guidepost Elementary Summer Camp or withdraw from specific weeks of the program, you can use this same form to request a withdrawal, so long as that request is submitted before the 10th of the month preceding the requested withdrawal (ex: withdrawal requests must be submitted by May 10th for all selected weeks of the June program). Please recognize that after the 10th of the month, Guidepost Montessori cannot issue refunds for any reason.

Dis-enrollment: Guidepost Montessori reserves the right, in its sole discretion, to discontinue the enrollment of any child, for any reason and at any time, with or without notice.

Non-Discrimination Policy: Guidepost Montessori does not discriminate on the basis of race, sex, national origin, disability, religion, or any other protected status. We make our best effort to accommodate the needs of every child who wishes to attend Guidepost Montessori. In the event that your child does have a disability or suspected disability, we will do a fully individualized assessment of your child in order to determine whether Guidepost Montessori possesses the necessary expertise and resources to provide for the best interests of your child, or, alternatively, whether the child may pose a health or safety threat to him/herself or others in our environment, or require a fundamental alteration of our program.

This is to acknowledge that I have read and understood, and agree to abide by all terms and agreements of this Enrollment Application. I agree that these terms are a condition of my child’s enrollment in Guidepost Elementary Summer Camp. I understand that after the 10th of the month, my tuition is non-refundable and non-transferable.

Step 7: Acknowledgments & Releases

Here at Guidepost we are obsessive about safety and security, and do everything we can to take amazing care of your child. However, as a parent, you know that there will always be some risks that are simply inherent in enrolling your child in school. A combination of state law and our internal risk management protocols requires that we include the following waivers.

Emergency Treatment Consent & General Consent to Attend School
I desire and consent for my child to enroll at Guidepost and to attend one or more Guidepost programs, including school day programs, after school programs, and other programs offered by Guidepost. I understand and have considered the risks involved, and I voluntarily and freely choose to assume these risks on behalf of myself and my child by allowing my child to attend school. Should the need arise, in the reasonable judgment of Guidepost Montessori, I hereby give my consent for any member of the Guidepost Montessori staff to authorize any acts reasonably believed necessary to provide for the health care of my minor child, including, but not limited to, that Guidepost Montessori may: (1) seek health care from any hospital or other institution, (2) retain any physician, dentist, nurse, or other person to care for my child (at my expense), or (3) consent to and/or authorize any health care or treatment, including administration of anesthesia, X-ray examination, performance of operations, or other procedures necessary to preserve the life, limb or well-being of the child, except for the withholding or withdrawal of life sustaining procedures.

I hereby, for myself and my child and our respective heirs, administrators, successors, and assigns, expressly release, waive, and agree to indemnify Guidepost (as defined above) from any and all claims or Losses (as defined above) that my child might suffer in connection with his or her participation in the school's programs and the administration or authorization of emergency medical treatment. This consent shall be governed by and interpreted in accordance with the laws of the State, without giving effect to principles of conflict of laws.

I ACKNOWLEDGE THAT I HAVE READ THE “EMERGENCY TREATMENT CONSENT & GENERAL CONSENT TO ATTEND SCHOOL”, FULLY UNDERSTAND IT, AND AGREE TO THE RESPECTIVE TERMS OF EACH.
